How to Interpret HOMA-IR Results

How to Interpret HOMA-IR Results

The HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) index measures how effectively cells respond to insulin after an 8–12 hour overnight fast.

HOMA-IR Score Classification Clinical Significance
< 1.0 Optimal (High Sensitivity) Excellent insulin sensitivity (HOMA-IR Minimal cardiometabolic risk.
1.0 – 1.9 Normal Range Normal insulin sensitivity (1.0 – 1.9). Healthy carbohydrate metabolism with balanced glucose regulation.
2.0 – 2.4 Borderline / Early Resistance Borderline zone (2.0 – 2.4). Tissues require increased insulin. Ideal window for dietary and exercise intervention.
2.5 – 2.9 Insulin Resistance Confirmed insulin resistance (HOMA-IR ≥ 2.5). Pancreas compensates by overproducing insulin (hyperinsulinemia). Medical consultation recommended.
≥ 3.0 Severe Insulin Resistance Marked insulin resistance (HOMA-IR ≥ 3.5). Highly associated with metabolic syndrome and type 2 diabetes risk.

HOMA-IR Calculation Formula

HOMA-IR = (Glucose [mg/dL] × Insulin [µU/mL]) ÷ 405

SI Units (mmol/L):
HOMA-IR = (Glucose [mmol/L] × Insulin [µU/mL]) ÷ 22.5

Frequently Asked Questions (FAQ)

What does a high HOMA-IR mean?
A score of 2.5 or higher indicates insulin resistance, meaning the pancreas must produce excess insulin to maintain normal blood sugar levels.
What blood tests are required for HOMA-IR?
You only need two fasting measurements from a single blood draw: fasting plasma glucose and fasting serum insulin.
Can insulin resistance be reversed?
Yes. In most cases, adopting a low-glycemic high-fiber diet, regular strength and aerobic exercise, and optimizing sleep can restore insulin sensitivity.
